What you will need
- Please bring all insurance information.
- Please bring your referral, if your insurance requires one. If you don't know if you need a referral, please call our office. If you don't have a referral at the time of your appointment, you will be rescheduled.
- Please bring all imaging and test results (ie: X-Rays, MRI's, EMG's) to help your physician best assess your condition. If you have had prior imaging performed, but you don't have the results, please make our office aware of these tests and where they were performed prior to your appointment.
- If you are a workers compensation patient, please bring your claim number, adjuster's name, and contact information.
