Your first visit to The Foot and Ankle Group establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. What you will need

  • Please bring all insurance information.
  • Please bring your referral, if your insurance requires one. If you don't know if you need a referral, please call our office. If you don't have a referral at the time of your appointment, you will be rescheduled.
  • Please bring all imaging and test results (ie: X-Rays, MRI's, EMG's) to help your physician best assess your condition. If you have had prior imaging performed, but you don't have the results, please make our office aware of these tests and where they were performed prior to your appointment.
  • If you are a workers compensation patient, please bring your claim number, adjuster's name, and contact information.

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you.
